Three children with medication on site did not have a parent signature authorizing the operation to administer the medication. NOTE: One parent signature was obtained during the inspection.

Staff member failed to use good judgement when she failed to follow first aid procedures and lightly puffed on a child's eye to alleviate discomfort which is not recommended and discouraged due to being unsanitary.

An infant was swaddled while sleeping in a crib.

Caregiver failed to use good judgement when she failed to properly read the name labeled on a child's bottle prior to feeding a child.

Caregivers do not sanitize cribs routinely prior to another infant using it.

Two out of 10 child immunization records reviewed were missing updated vaccinations.

A child received an injury at 10 am, showed obvious signs of being in pain by crying, not participating in activities, and inability to lay down for nap. Parent notification did not occur until 1 pm.

The caregiver in the 2 year old classroom did not know how many children were in care. Review of the children?s sign in and out log was also not up to date. There were 13 children signed in and only 11 children present. Note: This was corrected during inspection by the director when she signed 2 children out into another classroom during morning transitioning.

The bus #2 ( Tag number BT40277) would not start or turn over. The battery is dead. Bus # 4 ( Tag number U18784) the electronic child safety alarm is did not sound off. Note: This was corrected at inspection the director called maintaince and they got the bus back running again along with the alarm.
